Is the “were” in the second sentence correct?
Should it be “are” instead?

I'm assuming the context here is that in conversation, the other person mentioned where they're from and you simply forgot. In that case, "were" is perfectly fine. "Are" is fine as well, but is more awkward than "were". Here "were" is most accurate.
If this sentence is being asked without any context or as the start of a conversation, "were" would be suboptimal.

Both can be correct, but it depends on some context. Usually, "are" would be correct in most circumstances. If we're meeting for the first time, you would ask me "What state are you from?" However, if you're trying to recall which state I am from or if I have moved to a different state, you could ask me "Which state were you from again?"

Both are actually okay in this cirumstance! "Are" sounds more like you're just asking me where I'm from, whereas "were" sounds like you're trying to remember when I mentioned it before, in the past, but are still asking me the same thing.
