May I ask why 'right now' is better than ' now,,,?

'Now' can mean in the present time, but can also be used to show contrast between the past and the present. In this context it sounds like you previously had health issues.
> Example: I used to be very sickly as a child. I'm grateful that I don't have any major health issues now.
> Here, 'now' shows that in the past you were sickly, but you are not sickly at the present time.

'Right now' really emphasizes something in the present moment.
> Example: I'm grateful that I don't have any major health issues right now.
> This shows that you do not have any major health issues during the immediate present moment.
