How do you do?

Is it necessary to respond to the phrase "how do you do" with "how do you do?"
In an old movie, I heard the response: "Very well, thank you." Is this an outdated response?

In my experience words that are taught in textbooks and other language learning materials are not always what you hear in "real life." Greetings are an example of this. I have been in situations where people are meeting, and we end up speaking at the same time. So, while I am saying "Nice to meet you," to you, you might be saying "How are you?" to me. I think the best approach is to be flexible.

"How do you do?" is itself an outdated expression. Depending on where people are from, "How are you doing?" or just "How are you?" are more common, and usually the answer is just a quick summary of how you're doing and asking in return or continuing on the discussion.

I meant "how do you do" in the sense of "hello" and not in the sense of "how are you"

Yeah, I can't think of the last time someone said that to me in real life. People will understand what you mean, but it's not actually commonly used.
